Online nurse applications

The RN application collects your name, email, phone, city/county, ZIP code, license type and self-reported status, experience, visit interests (assessments, supervisory visits, or both), preferred service areas, availability, travel preference, insurance readiness, and acknowledgments. When you submit, these details are sent to CKK’s private application inbox for recruiting review and follow-up. Applications are currently reviewed in CKK’s private inbox. Email forwarding will be added after CKK’s dedicated email service is configured. Your reference number confirms receipt; it does not confirm credential approval or an assignment.

Application storage and access

Applications are stored using private server-side storage with an additional encryption layer. Application details are available through an authenticated CKK review inbox, not a public link. CKK uses the information to assess recruiting fit, contact you, and coordinate next steps. Archived applications remain stored until CKK removes them under its retention process. Contact CKK with questions about access, correction, or deletion.

Credential documents

This initial application does not collect Social Security numbers/cards, W-9s, CPR certificates, nursing license documents, TB results, or driver’s license images. CKK will provide separate instructions for secure onboarding after initial review. Please do not email these documents or put identity or medical information in an agency inquiry.

Agency visit planner

The agency visit planner creates a summary in your browser for you to copy. It does not submit a request or send email. Call 678-685-1801 to discuss your requirements. Do not include patient information in the planner.

Browser and hosting services

Application fields are not saved to browser storage. The application review inbox uses a secure session cookie that expires after 30 minutes. The website does not use advertising trackers. The hosting provider processes technical request information to operate the site, and short-lived request limits help prevent abuse. Application content is not intentionally written to application logs or sent to analytics.
